We would like to inform you that 2019-2020 REGISTRATION STARTS MONDAY, APRIL 15TH, 2019 Please note: registration will not be accepted if Health Records are not updated for the following grades: Pk-3 and Pk-4 - physical examination completed after March 2019. Kindergarten - physical examination, dental examination, and a vision examination completed after March 2019. 2nd Grade - dental examination completed after March 2019. 6th Grade - physical examination and a dental examination completed after March 2019. 9th Grade - physical examination completed after March 2019. 12th Grade - (2) doses of Meningococcal Conjugate Vaccine at the entry to 12th grade. It is very important that the parent completes and signs the Medical History portion of the physical form. Please find the attached Health ,Dental, and Eye exam forms.
