Medical Examination Requirements

If your child will be attending Pre-K, kindergarten, 6th and/or
9th grade, an updated health record must be made available at the time of
registration.

Next, print the following 2 forms, then bring it to your doctor's office or clinic. :

- HEALTH FORM

- DENTAL FORM

When you are finished, please return the forms to Universal School

Transcripts

Universal School graduates can request copies of their transcripts by completing the "Transcript Request Form". Please allow a maximum of one week after submitting request. There is a charge of $3.00 per transcript.
