1. Go to https://mytax.illinois.gov/
2. Enter username and password
3. Click “Sign On” and do not click “Sign Up Now”
4. Click “Individual”
located beneath “Account Type” 5. Click “Contribute to Invest in Kids” located beneath “I Want To”
6. Check acknowledgment box click “Next”
7. Choose Region: “Cook County”
8. Choose SGO: “Empower Illinois”
9. Enter Contribution Pledge: [suggested dollar amount no less than 2016 Illinois State tax, which can be found on the mytax.illinois.gov
website] 10. Enter Contribution Amount [match the same dollar amount as above]
11. Click “Submit”
12. Enter password when prompted
13. Print Page for Records
14. The Illinois Department of Revenue will email your Contribution Authorization Certificate (CAC) within a few minutes, and no later than 3 days of approval of your application.
STEP 3: DONATE – GO TO EMPOWER ILLINOIS TO PRE-PAY 2018 TAXES TO UNIVERSAL SCHOOL
1. To retrieve the Contribution Authorization Certificate:
a. Login to https://mytax.illinois.gov/
b. Click “CORRESPONDENCE”
c. Click “Invest in Kids Contributn Appl“ beneath Unread Letters
i. NOTE: after reading this letter, it will no longer be located under Unread Letters (but rather under “View Letters” and then “All” d. SAVE Contribution Authorization Certificate on your desktop (this will be needed in step 11 below)
e. Print the Contribution Authorization Certificate
2. Return to Empower Illinois Donation page to make your donation.

5. Click “I Have My Certificate”
6. Select Region: “Region 1” and click Next
7. Check “Individual”
and click Next 8. Check “Designate a School” and click Next
9. Select School: “Universal School – Bridgeview”
10. Enter your Contribution Authorization Certificate number, located on the second page of your Contribution Authorization Certificate letter XXX-XXXX-XXX and click Next.
11. Upload your Contribution Authorization Certificate letter received in your email from the State of Illinois (located on your desktop from step 1.d above).
12. Click “Make Online Donation Now”
13. Enter Contact Information
14. Enter Bank Account Payment Information
a. NOTE: This is a pre-payment of 2018 Illinois State Taxes
15. Click Confirm
CONGRATULATIONS!! Your 2018 Illinois State taxes are paid and will now be given to Universal School!!! Thank you!!!
